The notes were written on a single sheet of laid paper that bears a small round watermark. They are unsigned, though there is a red wax seal in the upper left corner. The verso of the paper bears a brief note, dated to March 9, 1790 and signed by Jonathan Conant.

A warrant directed to constable Levi Dodge, asking him to alert the community about a meeting of the voting committee to take place on July 1, 1802. The warrant was signed and sealed by members of the parish committee: John Conant, Rufus Putnam, and Ebenezer Trask. The verso of the warrant bears brief notes regarding this meeting held on July 1, 1802.
